First download and complete the entry form.  Submissions must be received by Monday, May 11, 2009.  You have several options for submitting your works of art to us.

Submit songs, videos, and posters by mail: 

You can mail your submission to the address below.  Include a completed and signed entry form.

• song entries should include a copy of your song on a cassette or DVD, and a copy of your written lyrics
• video PSA entries should include a copy of your script, as well as your video on DVD or VHS, or in MPEG or WMV format on a CD-ROM
• poster entries should be rolled up and mailed in a poster tube

 Drop off songs, videos, or posters at school:

Please check with your teacher or principal to find the drop off location in your school.  Rainbow Babies & Children’s Hospital staff will pick up all entries on the afternoon of Monday, May 11.

• poster entries do not have to be placed in a mailing tube, but take whatever precautions you deem necessary to protect your submission from damage
• all entries should be accompanied by a completed and signed entry form

Submit, songs, videos, or posters online:

To submit your song online:

• complete the online entry form
• convert your song to mp3 or WMA and make sure it is less than 10 MB in size
• upload the song and enter your song lyrics in the “song lyrics or video script” box
• name your song file like this:  “firstname_lastname_song”

 To submit your video online:

• complete the online entry form
• convert your video to MPEG or WMV and make sure it is less than 10 MB in size
• upload the video and enter your script in the “song lyrics or video script” box
• name your video file like this:  “firstname_lastname_video”

 If you designed a poster on a computer, you may convert it to a PDF and email it.  To submit a poster online:

• complete the online entry form
• convert your poster file to a pdf and make sure it is less than 10 MB in size
• upload the PDF of your poster on the entry form and name it  “firstname_lastname_poster”

To submit videos, songs, and posters  >10 MB in size, you should mail them in.
